Waiting Family Profile

Intro

Please answer all the questions contained within the FLO Waiting Family Profile form. Once you have submitted your form, a profile page will be created and posted on the public FLO web site. Profiles of families will be available to social workers through our A.R.C. (Active Recruitment Consulting) program. This means we don't wait for social workers to find you, we activity send your profile directly to them with your agency contact information.
Things to know before moving forward:
  • FLO reserves to the right to edit spelling, grammar, and length
  • FLO reserves to the right to remove any profile at our discretion without notification
  • FLO will not post a profile that contains incomplete information provided on this form
  • FLO will not post a profile of a family that does not have a completed, approved home study
  • FLO will not post a profile with the families contact information, you must be working with a placement social worker and have current and correct contact information
  • FLO does not guarantee that your profile will result in an adoptive or foster care family placement
  • FLO keeps your personal contact information confidential, only your agency contact information will be publicly viewable
  • FLO provides this profile services for family and social worker use, we strongly advise that you consult your placement agency regarding profile creation
